If you are reading this, chances are your Coach or Team Sportsmanship Liaison (TSL) asked you to review the materials on this page. One of the primary goals of PFA is to make players fall in love with the game of soccer and continue playing throughout their youth and into adulthood. As a mom or dad of a young soccer player, a significant part of your child’s decision to continue playing will be based on your encouragement and behavior before, during, and after practices and games. CONSENT AND LIABILITY WAIVER – RELEASE OF ALL CLAIMS:
I am the parent or legal guardian of registered player. As lawful consideration for my minor child being permitted to participate in the Patuxent Football Academy (“PFA”) Programs, I agree that neither my minor child nor I will make a claim against, sue, attach the property of or prosecute PFA and their agents, sponsors, independent consultants, and employees for damages for death, personal injury or property damage which my minor child may sustain as a result of my child’s participation in the PFA Programs. This release is intended to discharge in advance PFA and their agents, sponsors and employees from and against any and all liability, including negligent actions, arising out of or connected in any way with my minor child’s participation in the PFA programs, except for liability that may arise out of willful or wanton misconduct by Patuxent Football Academy.
I attest that I am eighteen (18) years old or older and that my child is physically fit and has no known medical conditions which prohibit participation in this sport. My child and I agree to follow all laws, rules, and guidelines regulating the conduct of the Patuxent Football Academy Programs. I understand and agree that my child and I are responsible for the operating condition of any and all sporting equipment provided by my child or by me for my child’s use, and I agree that my child and I will continuously inspect and maintain all equipment used, even if we have obtained any of the equipment of Patuxent Football Academy, their agents, sponsors, independent consultants, and / or employees.
I hereby give my consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor of medicine or dentistry or associated personnel provide the applicant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I understand treatment for injury will be based on information provided herein. I hereby authorize emergency transportation of the applicant/participant to a medical treatment facility should an individual listed above consider it to be warranted.
I FURTHER UNDERSTAND THAT SPORTS INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPATION IN SUCH SPORTING ACTIVITIES OCCASSIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF. KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE THAT MY MINOR CHILD AND I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS PATUXENT FOOTBALL ACADEMY, MSYSA, US CLUB SOCCER, USSF AND ITS AFFILIATED ORGANIZATIONS AND THEIR AGENTS, SPONSORS, INDEPENDENT CONSULTANTS AND EMPLOYEES WHO (THROUGH NEGLIGENCE OR CARELESSNESS) MIGHT OR OTHERWISE BE LIABLE TO ME, MY MINOR CHILD (OR OUR HEIRS OR ASSIGNS) FOR DAMAGES.
By Registering your child(ren) to participate in any Patuxent Football Academy Program you are responsible for prompt payment. PFA teams operate on a pay-to-play basis and an annual program of activities that begins on 1 August and ends on 31 July of each year. Registering on a PFA team is a commitment to pay the full amount of the annual program fees for that team as outlined in a team budget at the beginning of the annual program.
Fees paid to PFA are generally non-refundable. Under some circumstances participants may be eligible to apply for a refund. Requests shall be made to PFA in writing to the registered business address of PFA within fifteen (15) days of registration and payment.
By registering your child(ren) to participate in any Patuxent Football Academy Program you understand and agree that any failure to abide by the aforementioned rules and guidelines may result in disciplinary action against not only you as parent/guardian but also your child. You also agree to abide by the policies and procedures set forth by Patuxent Football Academy and the league(s) in which your child plays. Any such action may include but is not limited to those actions described above. It is furthermore understood and agreed that the Patuxent Football Academy specifically reserves the right impose disciplinary action above and beyond those actions described above as may be appropriate based upon the circumstances of the violation.
